HCH Primary Care Clinic is a unique healthcare facility, combining both clinic-based and home visit services, offering comprehensive primary and wellness care. Our philosophy revolves around prioritizing your needs, ensuring top-quality, effective, and all-encompassing primary care.
The journey of HCH Primary Clinic began in 2017 with a focus on delivering home visits to those who were homebound or faced transportation challenges. As our practice flourished, we expanded our services in 2019 to include office visits, allowing us to serve the communities surrounding our clinic.
Our overarching mission is to not only provide excellent care but also to educate our clients, empowering them to attain the highest level of health and wellbeing.
Central to our approach is the utilization of Nurse Practitioners who offer personalized and holistic care, tailored to meet each individual's unique needs.

I didn’t know what to expect after I arrived at the office. It’s not very often that I come across service that exceeds all expectations. The ENTIRE staff was so warm and inviting. I immediately felt comfortable. Dr. Anderson is very thorough. I didn’t feel rushed. She listened to my concerns, and addressed them accordingly.
